Vargo Chiropractic and Rehabilitation offers our patient forms online so they can be completed in the convenience of your own home or office.
DOWNLOADING INSTRUCTIONS
  • If you do not already have AdobeReader® installed on your computer, Click Here to download.
  • Click the "New Patient Forms" link below.
  • Save the forms to your computer before filling them out.
  • Open the form you saved on your computer and fill in all fields.
  • Print the completed forms and bring them in with you to your appointment or use the save as feature to save the completed form and email them to clincpaperwork@gmail.com. This will save you time on your first visit.
